Napoli took the lead when Dries Mertens beat the offside trap, before firing a well-taken finish past Alisson Becker.
Liverpool levelled things up via a less likely source in the second half, when Dejan Lovren scored his first goal of the season with a bullet header after a James Milner corner.
Despite dominating possession and having more shots than their opponents, Liverpool could not get the win they needed to seal early qualification for the knockout stages, meaning it will all come down to their trip to RB Salzburg in December.
